Maximizing the Effectiveness of Highly Active Antiretroviral Therapy: Is There a Role for Quadruple Regimens?*

چکیده

Although many highly active antiretroviral therapy (HAART) regimens using 3 drugs have demonstrated potent and durable antiretroviral effects, cohort studies have shown that treatment failure occurs in about 30% to 50% of patients. This failure rate prompts a closer look into the causes of treatment failure and how to modify or overcome these causes with new and more effective therapeutic regimens. Adherence to therapy may be the single most important factor affecting treatment outcomes with HAART. Factors affecting adherence include side effects of therapy, regimen complexity, total number of pills per day, pill size, food restrictions, water requirements, and dosing frequency. To improve adherence to HAART, dosing regimens must be simplified by reducing tablet load, removing food restrictions, and reducing dosing frequency. However, increased regimen convenience to achieve maximal adherence must be balanced with antiretroviral potency to achieve the lowest possible nadir. With a larger proportion of patients now beginning therapy with high viral loads and low CD4 cell counts, it is likely that many may not reach nadir on standard HAART regimens with 3 drugs. Therefore, maximized HAART regimens and 4drug (quadruple) regimens have been suggested and are being investigated. When given as initial HAART, quadruple regimens have shown promising preliminary results in recently completed studies and in ongoing comparative studies. Metabolic Abnormalities in HIV-Positive Patients Receiving Highly Active Antiretroviral Therapy

Background and Objective: Dyslipidemia has become a common problem in human immunodeficiency virus (HIV) disease, especially in patients on combination antiretroviral therapy. In this study we aimed to determine the prevalence of dyslipidemia and metabolic abnormalities in 2 groups of HIV infected patients receiving highly activ...

A randomized study comparing a three- and four-drug HAART regimen in first-line therapy (QUAD study).

BACKGROUND Evidence from randomized controlled trials supports the use of triple therapy. Research is required on the effectiveness of quadruple therapy in comparison to this and the relative effectiveness of specific highly active antiretroviral therapy (HAART) combinations. METHODS Antiretroviral-naive individuals (n = 53) with an HIV-1 viral load >100 000 copies/mL were randomized to recei...

Experiences with Helicobacter Pylori Treatment in Iran

Helicobacter pylori (H. Pylori) infection is currently recognized as the major cause of chronic active gastritis, peptic ulcer disease and mucosa associated lymphoid tissue (MALT) lymphoma and carcinoma of the stomach. Eradication of the infection will prevent the recurrence of the majority of such diseases. Different combined treatments have been tried in Iran for eradication of H. Pylori, but...

Treatment simplification in HIV-infected adults as a strategy to prevent toxicity, improve adherence, quality of life and decrease healthcare costs

Since the advent of highly active antiretroviral therapy (HAART), the treatment of human immunodeficiency virus (HIV) infection has become more potent and better tolerated. While the current treatment regimens still have limitations, they are more effective, more convenient, and less toxic than regimens used in the early HAART era, and new agents, formulations and strategies continue to be deve...
